GUIDE FOR AGENCIES & LANGUAGE SCHOOLS
Recognition of foreign-trained nurses in Germany: the process.
Anyone guiding nurses from non-EU countries to Germany takes them through a two-stage procedure, 16 competent state authorities and differing rules for documents. Here is the process that is the same in every state, and where the states differ.
- 01§ 40 PflBG
Two stages, everywhere
Every federal state splits the procedure: first the equivalence of the foreign training is determined, then the permit to use the professional title “Pflegefachfrau” or “Pflegefachmann” (registered nurse) is granted. Each stage has its own documents and fees; in North Rhine-Westphalia even different offices handle them.
- 02§ 40 Abs. 3a PflBG
Assessment or waiver
The default is an assessment based on the documents. If substantial differences are found, a compensation measure follows: the Kenntnisprüfung (knowledge exam) or an Anpassungslehrgang (adaptation course). The nurse may instead waive the equivalence assessment for good and go straight to the compensation measure. That saves the expert assessment and its cost, but it is a decision with consequences the authority has to explain.
- 03
Which state is responsible
The professional permit is valid nationwide, but where to apply is not a free choice. North Rhine-Westphalia and Baden-Württemberg require proof that the nurse intends to work in the state. Schleswig-Holstein accepts applications from abroad, with an employer's assurance among the first documents. Plan the state from the employer's location.

Documents: every state has its own rules
Copies, certifications and translations are a state matter. Lower Saxony wants simple copies only and destroys submitted documents after intake. North Rhine-Westphalia accepts translations only made from the original or from a copy certified in Germany. Berlin rejects translations made outside the EU. Not knowing these differences means requests for more documents and lost months.
Agencies can file on a nurse's behalf in several states: the forms in North Rhine-Westphalia and Lower Saxony include a power of attorney with fields for a company. Berlin does not offer this route.
- 05§ 16d Abs. 3 AufenthG
German: B2, but only later
Where the states name a level, it is B2. The language certificate is only required before the professional permit, not when applying for equivalence. Recognition and the language course can therefore run in parallel. With a recognition partnership (Anerkennungspartnerschaft) entry is possible from A2; until the permit is granted the nurse works in an assistant role.
NurseBridge knows the checklists of all 16 states, checks each case against them and requests missing documents from the nurse. Recognition applications for NRW, Lower Saxony, Schleswig-Holstein, Hamburg, Bremen and Baden-Württemberg are filled from the confirmed profile. Your team decides what is submitted.
